How to apply for a STAP voucher
- Print out HHSC STAP application Form 3906. (If you have trouble viewing this PDF, use this link)
- Page 1: Fill out your information
- Page 2: Attach your proof of residency.
- Page 2-3: Select the Device type aligned with your disability.
- Take Page 4 of your application to a professional to certify your disability.
- Authorized certifiers include:
- Physician = your doctor

HHSC contracted IL Specialist (at a center for independent living)
- Social worker
- Speech pathologist (aka speech therapist)
- Audiologist
- Optometrist
- TWC rehabilitation counselor
- State-certified education teacher of:
- special education
- deaf or hard of hearing
- visually impaired
- speech impaired

- If you have not received the voucher in 2 months, follow up on your application with HHSC STAP department by email (dhhs.phones@hhs.texas.gov) or calling 1-512-438-4880.
